I am trying to access my pc with an Ipad over the internet. Works great on the lan, but I want to do it over the internet.

I have read your guide, but it is Mac only.

I have ultravnc installed on my pc, I have a domain set up at dyndns.org ( carrycam.dyndns.org).

I cannot make it work...the consistent error message is Disconnected - error connecting to server.

What are the clear steps to set this up on a Windows 7 PC? Am I missing something?

Again, works fine over the lan.

Check your port forwarding. There are some useful pages to scan your ports online. Try these:
